THIS IS NOT DENTAL INSURANCE. Cannot be combined with other insurance or discount plans. Membership plan is specific to one person and cannot be transferred. Covered services not used within the specified time period will not be carried over or transferred. Services will be provided at this location only. Membership plan will auto-renew each year on the renewal date. If a patient presents with periodontal infection, a personalized treatment plan will be presented at an additional cost. The perio plan is strictly for preventive maintenance care.

Our list of accepted insurance policies is ever-growing! We invite you to please call our office at (972) 779.0300 to verify accepted insurance. We will diligently work with your insurance provider to process your dental expenses and get you the fullest amount of benefits your provider allows. Please note, however, that not all dental procedures and costs may be covered by insurance. Any remaining balance not covered by the insurance provider becomes the responsibility of the patient. Please see payment methods below for ways we work with you to pay co-pays and remaining balances.
